Spring Report - Submission Page


 

INSTRUCTIONS:   Observations of any changes in water quality, clarity, discharge or flora or fauna should be immediately recorded and reported.  This information will be used to provide early identification of problems and to help determine the causes.  Examples of events that should be reported include: unusual cloudiness or discoloration, unusual odor, reduced flow, dead fish or other animals, loss of flora and fauna, or presence of exotic aquatic plant growth in the spring or spring run. 

Spring Name
Date and Time
of observation
Precise location of the observation (provide enough information so another person can find the site)
Description of the observed condition
Duration of event
What samples were collected (water, flora, fauna, etc) and by whom (provide contact name, phone number, address and email address) and a copy of any report they provided
Describe any activities or conditions in the spring's recharge area that may have caused the event
Report provided by (name, organization, address, telephone number and email address)
